In oral cavity, there is harmonious environment between tooth, muscles and joint (Temporomandibular Joint – TMJ). Any change in any of components, lead to pain and when the pain is of muscle origin it is called “Myofascial Pain”.

There are four cardinal signs and symptoms of myofascial pain dysfunction syndrome (MPDS)

  • 1.Pain
    • Either localized or referred to head, neck, shoulder or back.
    • Usually unilateral, dull and persistent in nature.
  • 2. Tenderness
    • At the affected joint and involved muscle area. Usually it is near (front portion of) ear.
    • Tenderness is best palpated at the insertion area of affected masticatory muscles (muscles helping in chewing)
  • 3. Clicking / popping noise in the joint
    • It may occur while opening or closing.
    • Single or multiple.
    • Unilateral or bilateral.
  • 4. Deviation of the jaw
    • While opening affected muscles restrict movement of mandible (lower jaw) and hence jaw seems deviated to affected side while opening mouth.

As muscles, teeth and joints are in harmony, any disturbance in this balance may lead myofascial pain.

Medicines and exercises are parts of the first line of management. Occlusal splint (transperant plate on teeth) is fabricated when required.

As components of stomatognathic system (teeth, bone and muscles) are dynamic, any event in future, impairing the balance may trigger MPDS again.
